# Employee productivity scorecard template

Prepared by Neebix Editorial Team · October 2, 2026

Use a small, complementary set of role-specific measures. This template has no preset targets, metric weights, or universal performance score. Keep private employee and client records in their authorized systems.

## Scope

- Role or comparable team:
- Work type and complexity segment:
- Review period and measurement window:
- Output definition and acceptance criteria:
- Data sources and review owner:
- Representative baseline period:
- Dependencies outside the team's control:

## Complementary measures

| Layer | Chosen measure | Definition and denominator | Source | Baseline | Current period | Context / limitations |
| --- | --- | --- | --- | --- | --- | --- |
| Output | | | | | | |
| Quality / rework | | | | | | |
| Delivery / cycle time | | | | | | |
| Customer or business outcome | | | | | | |
| Workload / available capacity | | | | | | |

Output per hour = useful output ÷ appropriate labor hours.

Accepted output per hour = outputs accepted without significant rework ÷ appropriate labor hours. Define the acceptance window and count each output consistently.

Rework rate = outputs requiring substantial correction ÷ total outputs × 100. Agree the correction threshold before comparing periods.

The article's on-time completion rate uses work completed by deadline ÷ total completed work × 100. If overdue unfinished work matters, review the backlog separately or choose a due-work denominator and document the difference.

## Diagnostic context, only where relevant

- Workload and capacity changes:
- Missing inputs, approvals, or scope changes:
- Meeting load, focus opportunities, or work fragmentation:
- Application patterns relevant to a defined question:
- Employee/team explanation:
- Data-quality or comparability concerns:

## Monthly review

1. Review output and business outcomes.
2. Check quality, rework, and delivery.
3. Review workload and dependencies.
4. Consult activity context if an important change remains unexplained.
5. Ask the team what changed.
6. Agree one action, an owner, and a follow-up date.

- Agreed action:
- Owner:
- Follow-up date:
- What evidence would show the action helped:
- What remains uncertain:
